Practice Policies and Information


 

In-Network Insurance

We are participating providers for the following PPOs: Aetna, BCBS, Cigna, Humana, Health Link, Medicare, and United Health Care. In addition, we participate with Illinois Worker’s Compensation. This means we have agreed to accept discounted rates for our services, and agree not to bill you beyond the “allowed amount.” We ask that you know your deductible (the amount you need to pay out of pocket before benefits begin) and your co-payment so that you may pay this amount at the time of service. We are happy to bill your insurance carrier directly. However, we must have a current copy of your insurance card(s) and up-to-date registration information on file.
You are financially responsible to the Practice for any covered or non-covered services as defined by your insurer. It is your responsibility to check with your insurance company to determine if Innovative Pain Specialists, LLC is an “in-network” provider before your visit. Should there be a problem with reimbursement, or should you receive a reimbursement check rather than the practice, it is your responsibility to contact the insurance company to rectify the problem as soon as possible. Your account may be billed if payment is not received within 60 days of rendered service.

Non-Participating Insurance

If we are not a preferred provider in your PPO or HMO Plan, we are still able to take care of you. You are expected to pay at the time of service. For most plans, there is an “out-of-network” allowance, and we are happy to provide you with the necessary forms to submit to your insurance company. You will be mailed a check from the insurance company for any allowed amounts.

Self-Pay / No Insurance

We are happy to care for patients, even if they do not have health insurance. We understand that health care costs are significant and we offer a special discount for those without insurance or who have financial hardship. In addition, if you have a high deductible and do not wish to use your insurance company, we will accomodate you. We accept cash, personal check, Visa, MasterCard, Discover and American Express. Payment is expected in full at the time of service.

Missed Appointments

Appointments are scheduled for 45 minutes for initial consultations, and 15-30 minutes for follow-up visits and procedures. If you need to cancel a scheduled appointment, it is imperative that you cancel with at least 24 hours advance notice or you will be charged $100 for a missed initial consultation and $50 for a missed follow-up visit or procedure. Please be aware that you are fully responsible for such charges and insurance companies do not pay for missed appointments.

Billing / Payments

Charges are based on time involved, the urgency / complexity of the problem, your medical history, and the issues discussed.
Cash, Checks or MasterCard/Visa/Discover/AmEx are accepted.
There is a $30 fee for returned checks.
If your account has not been paid for 90 days and arrangements for payment have not been agreed upon, other means may be used to secure payment such as a collection agency or small claims court.

Pharmacy / Medication Refills

Every attempt will be made by the physician to refill ALL your medications at the time of your scheduled follow-up visit. It is your responsibility to be aware of your remaining medication and take it as prescribed. Medications will not be refilled when the office is closed. We will work to refill medications as soon as we can, however, please allow up to 72 hours for a refill. Please keep an updated list of your medications with you at all times, and bring it to all your appointments.

Urgent Care

If you are ill or in need of urgent care, the physicians at Innovative Pain Specialists will make every attempt to see you within 24-48 hours. Slots are usually kept open in the schedule for this purpose. Please call for an urgent appointment as we do not take walk-in visits. Also, be aware that you may be asked to wait for a short period if others are already scheduled.

Contacting your Provider

Non-urgent calls and messages for the physicians are usually returned within 24 hours. If for some reason you have not heard back from us, please call again, in that, voice mails, emails, and messaging can be faulty. If your message is urgent, please discuss this with the receptionist or medical assistant. Lastly, you may reach the on-call physician outside of office hours any time for extremely urgent matters at (847) 596-0911. FOR ANY EMERGENCIES PLEASE DIAL 911.
